Welcome to the Life Church International online applications.
To submit an application for credentials, church affiliation with Life Church International, start by selecting the name of the type of application you require. Enter your information and answer the question given.
An application does not have to be completed in one sitting. If you are unable to finish your application for any reason, exit via the cancel button. This will save all your details except the screen you are currently adding information to.
To continue with a partially completed application, enter your name and date of birth into the first three fields. Your previously entered information will be displayed, allowing you to proceed.
Applications for Credentials
All applications for credentials with Life Church International must enter their details online.
Passport-size photos in .jpg format are mandatory to complete the application. You will need this for yourself, your spouse (if she is applying for credentials), and a digital photo of the two of you. If you are also affiliating your church, you should provide a digital photo of the premises from which you operate.
